A Clinical Review on Biliary Stones and Clonorchiasis in West Gyeongnam Area



Abstract
We have operated on 128 patients who had the biliary stones during the last 3 years, atr the Department of Surgery, Gyeongsang National University Hospital.
@ES The result of clinical analysis of these patients were summarized as follows;
@EN 1) The association rate of clonorchiasis in all biliary stone disease was 22.7% (29 of 128 cases), which was higher than that in other areas.
2) They comprised 50 men and 78 women with sex ratio of 1:1.6. But among the patients who had the biliary stones associated with clonorchiasis, the male to female ratio was 1.6:1.
3) The most common symptom and sign were RUQ pain (90.6%) and RUQ tenderness (79.7%). The subjective symptoms and objective signs in the patients with a clonorchiasis were not different from those in the patients without a clonorchiasis.
4) The leukocytosis was present in 49 cases (38.3%), elevated SGPT in 71 cases (55.5%), elevated alkaline phosphatase in 65 cases (50.8%), elevated SGOT in 51 cases (39.8%).
5) The accuracy of the diagnostic methods for stone detection was 100% in PTC, 90.3% in ERCP, 88.9% in CT, 88.0% in U.S.
6) The stool examination showed the highest diagnostic rate for detecting the clonorchiasis.
7) As whole, 36.7% of patients had CBD stone and 32.0% of patients had Gall stone and 31.3% of patients had intrahepatic stone. But, 48.2% of patients who had biliary clonorchiasis had gallstone.
8) Cholecystectomy was most common operative procedure in the patients associated with clonorchiasis because the frequency of GB stone was high and cholecystectomy with P-tube choledochostomy was most common oeprative procedure in the patients
not
associated with clonorchiasis because the frequency of CBD and IHD stone was high.
9) The complication rate was 24.2% and pulmonary complication was developed most frequently. Residual sotne was developed in 7 cases (5.5%).
10) The amount of daily drainage of bile from T-tube in postoperative period increased significantry in biliary stone diseases associated with clonorchiasis.
According to the above resuits, characteristically, the incidence of CBD nad IHD stones and association rate of clonorchiasis in biliary stones in Weat Gyeongnam area are higher than those in othe rareas. But, of the biliary stones associated
with
clonorchiasis, the GB sthoes were most common. So, clonorchiasis did not seem to contribute on the formation of CBD and IHD stones.
